State law requires students to have updated physical and dental examinations at the following intervals:

Updated health/physical assessments:

• upon original entry into school
• when starting 6^th grade
• when starting 9th grade

Updated dental assessments:

• Upon original entry into school
• when starting 3^rd grade
• when starting 7^th grade

It is also recommended that Parents/Guardians submit a copy of updated health and dental assessments every year after their students are seen by their physician and/or dentist, so that the school nurse will have current information.

The physical and dental examinations of the student may be completed during the summer or any time within one year prior to the start of the school year, using the Physical and Dental Examination Forms. Exams before September 2021 or after September 6, 2022 You may download them below.

Medical and Dental Forms

The Pennsylvania Public School Code requires all public schools, including charter schools, to determine that a student has been immunized in accordance with state law prior to enrollment in and attendance at school. The school nurse is responsible to review each student’s immunization records to ensure that he or she meets state requirements.

A student whose parent or guardian fails to provide complete and accurate immunization records to the school nurse risks exclusion from school until state immunization requirements are met.

If a student is exempt from immunization, he or she may be excluded from school during an outbreak of a vaccine-preventable disease.

Required vaccinations

If your child needs to keep medication and/or equipment in our Nurse’s office, an updated doctor’s order for each medication and/or piece of equipment (Med-1 Form) is required.  Please note:

• One Med-1 form per medication is needed. These forms are valid for one school year.

• Medication should be given at home whenever possible. 

• Medication at school should only be considered when it is not possible at home. An example would be a rescue medication (like an inhaler or epi-pen) or those students who may require a midday dose, in addition to those you administer at home. 

• No medication will be administered to your child without: 

  • (1) a recent doctor’s order* 

  • (2) up-to-date prescription label on the original medical container/box

  • (3) medication that is hand-delivered to the School Nurse the first day of the new school year or whenever medically necessary 

• Medically-necessary equipment should have:

  • (1) a recent doctor’s order*

  • (2) associated labels and original containers

• No student should be taking medication on their own in the classroom or have any medication on them. 

• All medications are handled through the Nurse’s office. 

• Carry your own permits (grades 6-8) are for inhalers and epi pens. Those exceptions still require all the forms and steps mentioned above and need to be approved by the nurse’s office.

*THE MED1 FORM MUST BE DATED AFTER JULY 1, 2021 BY A DOCTOR. MEDICATION THAT IS NOT ACCOMPANIED BY THE MED1 FORM OR IS NOT PROPERLY LABELED WILL NOT BE ACCEPTED.

If your student has a chronic condition (such as asthma or a peanut allergy), a yearly update is recommended (either a letter by the student’s medical provider or an updated exam form).
